Traits

Third Arm.Sam has a third arm that allows him to take an additional bonus action on each of his turns to use an item or make a scalpel attack.
Mutant Surgeon.Sam has advantage on Wisdom (Medicine) checks. When he restores hit points to a creature, he can also choose to inflict a minor mutation (DM's choice).

Actions

Multiattack.Sam makes two Scalpel attacks.
Scalpel.Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 5 (1d4 + 3) piercing damage plus 7 (2d6) necrotic damage.
Emergency Amputation.Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 10 (2d6 + 3) slashing damage. If the target is a creature, it must succeed on a DC 15 Constitution saving throw or have its speed reduced by 10 feet and take 5 (1d10) necrotic damage at the start of each of its turns until it receives magical healing or a DC 15 Medicine check.

Saw-Bones Sam

Title: The Mutant Surgeon
Role: Sam is a man whose body is a testament to the dangers of the Wastes. He has a third arm growing from his chest and one eye that glows with a sickly green light. He is the town’s only doctor, specializing in treating "Void-Blight" and performing "emergency amputations" for scavengers who get caught in traps.

Secret Motivation: Sam is secretly working for the Whispering Lich, Kel'Thuz. He uses his "clinic" to collect biological samples from the town’s mutants, which he then sends to the Lich to help him create a "Perfect Vessel" for his soul. Sam is also "experimenting" on his patients, trying to create a new race of "Void-Soldiers" that can survive in the deepest parts of the Wastes.

Adventure Hook: A group of Sam’s "success stories" (mutants with Void-powers) escapes the clinic and starts terrorizing the town. Mordred hires the party to "put them down" and find out what Sam is really up to.
